Preparing for your first appointment

Below, we’ve outlined step-by-step instructions you’ll need to take leading up to your first appointment and what to bring to your first telehealth session. This will help us provide quality behavioral health services, assure seamless payment by your insurance provider and help make you as comfortable and prepared as possible during your first appointment.

Call to Schedule Your Appointment

To schedule your first telehealth appointment, call 866.852.4001.

A video call will be part of the scheduling process. This will allow us to take a screen shot of your driver’s license and insurance card. We use a video app called BlueJeans. If you would like to test your connection before calling, click on this BlueJeans Test Link and download the app to your device. When the test window opens, you’ll see Jean the BlueJeans cartoon parrot who will help you check to make sure your microphone and video are working properly.

Complete the Paperwork.

At Least 24 Hours Before Your Appointment.

In order to provide the highest quality service, you will need to tell us more about the concerns you would like addressed, your personal and family history, as well as any medical conditions or current medications. You will also sign a consent to treatment and choose your method of payment for any co-payments and deductibles for your sessions.

Gather your information together before you start. The online form is quite comprehensive and could take approximately 30 minutes to complete. Please complete all forms to the best of your ability.

To start, select the tab for your state and click on the button below which lists the age of the person being treated. If you are receiving services through your Employee Assistance, Church Assistance or School Assistance Program, please use the forms that are labeled EAP on the corresponding tab for your state. A new window will open asking you to set up an account so your form can be saved and completed later, if needed.

Obtain Insurance Preauthorization.

Before your first appointment.

When you called in to schedule your appointment, our staff verified that your insurance covers telehealth outpatient behavioral health services. However, a preauthorization may be required by your provider. Please contact your insurance company to verify benefits and secure any preauthorization requirements. If a required authorization is not obtained, you will be responsible for payment of services.

Choose an appropriate location, room or setting for your appointment where you can talk freely without being overheard or interrupted. We will be sure to do the same on our end.
